1. I understand the nature of this program is education and not medical diagnosis or treatment

2. I have read the refund policy in full
a)No refunds for Muse headband (you may contact Muse directly for refund
b)No refunds for Monthly Payment Participants (no exceptions)
c)Refunds given 100% prior to initial assessment for clients who pay in full only and if electrodes were not sent yet.

d)After initial assessment occurs refunds given if you meet the following requirement:
-Participant must show they used their neurofeedback plan DAILY for at least 2 weeks to receive a refund (no refunds for those who did not try to do the training)
-Refunds must be requested before 30 days, no refunds issued after 30 days.
-Refunds will be issued minus a non-negotiable $300 administration fee (to cover initial visit, use of program, and electrodes/case).


3. I understand this payment/subscription will grant access to the program for 3 months only after which my access to neurofeedback program will be stopped unless additional payment is given.

4. I understand that I should be under the care of a mental health care provider either counselor, psychologist, psychiatrist or pastor/spiritual worker while participating and this program in case trauma related memories occur. At home neurofeedback and Dr. Irene's online coaching is not a substitute for being under the care of a certified medical provider in my state.

5. As the participant, I recognize and acknowledge that there are risks of physical injury and I agree to assume the full risk of any injuries (including death), damages, or loss which I may sustain as a result of participating in any and all activities arising out of, connected with, or in any way associated with wellness activities. I acknowledge that participation in these activities is voluntary.

6. I, on behalf of myself , do hereby fully release and discharge Mind Door LLC , Dr Irene Luc and their agents, contractors, and employees from any and all liability, claims, and causes of action from injuries or illness (including death), damages or loss which I may have or which may accrue to me on account of participation in suggested wellness activities. This is a complete and irrevocable release and waiver of liability. Specifically, and without limitation, I, on behalf of myself, hereby release the Released Parties from any liability, claim, or cause of action arising out of the Released Parties’ negligence. I, on behalf of myself, covenant not to sue the Released Parties for any alleged liabilities, claims, or causes of action released hereunder.

7.I further agree to indemnify and hold harmless and defend the Released Parties from any and all claims resulting from injuries or illness (including death), damages, or loss, including, but not limited to attorneys’ fees, sustained by me arising out of, connected with, or in any way associated with my participation in wellness activities.
